Smith Therapy wants to make referring your patients easy for you. We will take care of scheduling by quickly following up with your patients once the referral has been forwarded.

Throughout the patient’s therapy, Smith Therapy will provide progress and important information back to the physician per your preference; fax, e-mail or phone. We recognize your practice doesn’t have time to manage another resource. That is why we have become the most referred physical therapy group in Greenville amongst area physicians.

When completing a referral, please include:

Date of the referral
Patient’s name
Diagnosis
Area of treatment, frequency and duration
Instructions and/or precautions
